Mi Via

The Mi Via (My Path) program is a person-centered, community-oriented approach to deliver services that assist eligible adults with Intellectual and Developmental Disabilities (IDD) to participate as active members of their communities. The program is intended to provide a community-based alternative to institutional care that facilitates greater choice, direction and control over the support for the participant.

 

The Mi Via program emphasizes that people with IDD:

  • Have control over their lives as much as possible.

  • Have opportunities to use resources in ways that enhance their lives and help them participate in their community.

  • Have value, potential, and will be viewed in terms of their abilities.

  • Have the right to participate and be fully included in their communities.

  • Have the right to live, work, learn, and receive services and supports to meet their individual needs in the most integrated settings possible within their community.

 

Great Livin’ provides the following Mi Via services and supports:

  • Customized In-Home Living Supports (Family Living)

  • Community Direct Supports (Community Access)

  • Adult Nursing Services

  • Behavior Support


Great Livin’ will customize services to meet the needs of caregivers and those they serve.

DD Waiver

The DD Waiver Program is a person-centered, community-oriented approach to deliver supports for people with intellectual and developmental disabilities (IDD). The program is designed as an alternative to institutional care to provide services and supports that assist eligible adults with Intellectual and Developmental Disabilities (IDD) to participate as active members of their communities.

 

The DD Waiver program emphasizes that people with IDD:

  • Take charge of their lives as much as possible.

  • Have options allowing the use of resources that enrich their lives and help them engage in their community.

The DD Waiver program provides an array of residential, in-home, community, employment, therapeutic and family support services. DD Waiver services are intended to augment, rather than replace, other supports an individual may have, such as medical services through the Medicaid State Plan, natural supports and other community services.

 

Great Livin’ provides the following DD Waiver services and supports:

  • Customized In-Home Living Supports (Family Living)

  • Community Direct Supports (Community Access)

  • Adult Nursing Services

  • Behavior Support

ICF-IDD

The Intermediate Care Facilities for individuals with Intellectual Disability (ICF-IDD) program provides comprehensive and individualized health care and rehabilitation services to individuals to promote their functional status and independence. ICF-IDD is an alternative to home and community-based services waiver programs for individuals at the ICF-IDD level of care. The program provides active treatment in the least restrictive setting and includes all needed services for individuals whose mental or physical condition require services on a regular basis.

 

Great Livin’ provides the following ICF-IDD services and supports:

  • Community integration

  • Traditional neighborhood setting

  • Community integrated day services

  • Adult Nursing Services

  • Therapies (OT, PT, SLP, etc.)

  • Behavior Support

State General Funds

Great Livin’ offers Behavior Support Consultation services subsidized from New Mexico State General Funds. These services are for individuals who have not been allocated to any Waiver and are in need of an assessment and behavior support plan. This service includes a few follow-up sessions but is not intended to provide on-going care.
